Core Connect provides staffing and related administrative support for CORE and other behavioral-health services. Core Connect does not bill Medicaid directly and provides services under or on behalf of participating healthcare and behavioral-health agencies.

Agency and Business Information

If you represent a healthcare or behavioral-health agency, we may collect your name, title, agency affiliation, business contact information, staffing needs, contractual information, and records concerning services performed.

Service and Client Information

When necessary to arrange, coordinate, document, or support services for an agency, Core Connect may receive limited information concerning individuals receiving behavioral-health services. This information may include contact information, eligibility or service information, scheduling information, service plans, documentation, and other health-related information provided by or on behalf of the agency.

Core Connect seeks to collect, access, use, and disclose only the information reasonably necessary to perform its contracted responsibilities.

3. Protected Health Information

In some circumstances, Core Connect may receive or access protected health information while performing services on behalf of a healthcare or behavioral-health agency.

When Core Connect acts as a business associate or subcontractor under the Health Insurance Portability and Accountability Act (“HIPAA”), Core Connect uses and discloses protected health information only as permitted by applicable law, its written agreement with the relevant agency, and the agency’s privacy practices.

The healthcare or behavioral-health agency responsible for an individual’s care should provide that individual with its Notice of Privacy Practices. Requests involving access to, amendment of, or restrictions concerning a clinical or agency record may need to be directed to that agency. Core Connect will reasonably assist the agency in responding to such requests when required.

If you provide your telephone number and consent to receive text messages, Core Connect may send you non-marketing communications related to your inquiry or relationship with us. These may include:

Core Connect does not sell mobile information. Mobile information will not be shared with third parties or affiliates for their own marketing or promotional purposes. Text-message opt-in information and consent will not be sold or shared with third parties for marketing purposes. Core Connect may provide limited information to vendors that process and deliver messages solely on our behalf, subject to appropriate confidentiality and data-protection obligations.

Participating Agencies

We may provide information to healthcare or behavioral-health agencies for staffing, credentialing, assignments, scheduling, supervision, service coordination, documentation, compliance, quality assurance, and related purposes.

Service Providers

We may use vendors that provide hosting, communications, scheduling, credentialing, background screening, payroll, payment processing, data storage, security, analytics, or other operational services. These providers may use information only to perform services for Core Connect or as otherwise permitted by law.

Authorized Healthcare and Service Participants

When permitted by the applicable agency, contract, authorization, or law, information may be provided to healthcare professionals, care coordinators, insurers, government healthcare programs, or other authorized participants involved in service delivery or administration.

Legal, Regulatory, and Safety Purposes

We may disclose information when required or permitted by law, regulation, subpoena, court order, audit, or government request, or when reasonably necessary to protect the rights, safety, or security of an individual, an agency, Core Connect, or the public.

Core Connect uses reasonable administrative, technical, and physical safeguards designed to protect information against unauthorized access, use, alteration, loss, or disclosure.

No method of electronic transmission or storage is completely secure. Accordingly, Core Connect cannot guarantee the absolute security of information.

9. Your Choices and Requests

Subject to applicable law, contractual obligations, and record-retention requirements, you may ask Core Connect to:

  • Provide information about the personal information we maintain about you

  • Correct inaccurate personal information

  • Delete personal information that Core Connect is not legally or contractually required to retain

Requests concerning records maintained by a healthcare or behavioral-health agency should ordinarily be submitted directly to that agency. If Core Connect receives such a request, we may refer or transmit it to the appropriate agency.
